Hip arthritis in dogs is one of the most common causes of hind-limb lameness and mobility decline in older pets. It develops gradually—often beginning with subtle stiffness after rest—and progresses to chronic pain that limits everyday activities. While it cannot be cured, a combination of weight management, controlled exercise, veterinary-directed pain relief, and home modifications can meaningfully slow progression and maintain quality of life. Early recognition and a multimodal management plan make the biggest difference.
Hip dysplasia and hip arthritis are related but distinct. Dysplasia is a developmental joint malformation—the ball and socket grow improperly, creating joint laxity and abnormal contact. Arthritis is the degenerative joint disease that results from that abnormal wear. A dog with hip dysplasia may develop arthritis over months or years, but not all hip arthritis stems from dysplasia—injury, obesity, and age-related wear can also cause it independently.
Dysplasia is typically identified through physical examination and radiographic screening, often in young dogs. Arthritis develops later as the joint degenerates. Treatment strategies overlap but are not identical: dysplasia management may include surgical correction of joint mechanics, while arthritis management focuses on pain control, weight, muscle maintenance, and activity modification.
Symptoms develop gradually and are often initially attributed to "slowing down with age." Because dogs tend to mask pain, the changes you notice may represent a more advanced stage than what the joint shows internally.

If your dog shows even early signs, a veterinary evaluation is worthwhile. Lameness and mobility changes can also signal neurologic disease, other joint conditions, or abdominal pain—accurate diagnosis guides the right treatment.
Diagnosis typically involves a combination of history, examination, and imaging.
History and observation. Your veterinarian will ask about changes you have noticed—stiffness timing, activity level, pain episodes, and any triggers. Observing your dog's gait across the exam room provides valuable information about weight-bearing, stride, and symmetry.
Orthopedic examination. Palpation of the hip joints checks for pain on extension, range of motion, crepitus (a grinding sensation), and muscle mass symmetry. Both hips are usually compared. A concurrent neurologic evaluation helps distinguish hip arthritis from lumbosacral disease or degenerative myelopathy, which can produce similar hind-limb signs.
Radiographs (X-rays). The primary imaging tool. X-rays reveal joint space narrowing, osteophyte formation, femoral head remodeling, and acetabular changes. Sedation or anesthesia is usually needed for proper positioning and a diagnostic-quality study.
Advanced imaging. CT or MRI may be recommended for complex cases—when surgical planning requires detailed anatomy, when concurrent soft tissue injury is suspected, or when radiographic findings do not match clinical severity.
Hip arthritis cannot be reversed, but a coordinated multimodal approach can significantly reduce pain, preserve muscle and mobility, and maintain quality of life.
Weight management. The single most impactful intervention. Reducing excess body weight decreases mechanical stress on the joints and lowers systemic inflammatory load. Even modest weight loss—as little as 6–8% of body weight in overweight dogs—can produce visible improvements in mobility and comfort. Your veterinarian can help set a target weight and feeding plan.
Controlled exercise. Short, frequent walks on flat, even surfaces maintain joint mobility and muscle mass without overloading the hips. Swimming and underwater treadmill therapy provide low-impact conditioning that builds strength with minimal joint stress. Exercise duration and intensity should be guided by your veterinarian based on your dog's current pain level and functional capacity.
Rehabilitation therapy. Veterinary rehabilitation may include passive range-of-motion exercises, gentle stretching, therapeutic massage, cold and heat therapy, and targeted muscle-strengthening protocols. These are most effective as part of a consistent, long-term program.
Home modifications. Non-slip flooring or rugs on smooth surfaces, ramps instead of stairs for accessing cars or beds, orthopedic beds that support the joints and make rising easier, and elevated food and water bowls to reduce postural strain during meals.
Never give human pain medications—ibuprofen, acetaminophen, naproxen, and aspirin can be toxic to dogs, even in small doses. Always consult your veterinarian before starting any pain management.

Your veterinarian will tailor the plan to your dog's severity, concurrent health conditions, and response to treatment. Regular rechecks—typically every 3 to 6 months for stable cases—allow adjustments as the disease progresses.
Surgery is considered when medical and rehabilitation management no longer controls pain adequately, when quality of life is declining despite multimodal treatment, or when the degree of joint damage warrants structural correction.
Total hip replacement (THR). The most definitive surgical treatment. The damaged femoral head and acetabulum are replaced with prosthetic components, restoring near-normal joint mechanics. THR provides the most complete pain relief and functional recovery but is a major procedure that requires careful patient selection, experienced surgical teams, and a structured postoperative rehabilitation program.
Femoral head ostectomy (FHO). The femoral head is removed, and the body forms a fibrous "false joint" supported by surrounding muscle. FHO eliminates bone-on-bone pain and is most effective in smaller dogs, cats, and dogs that are not candidates for total hip replacement. Functional outcomes depend heavily on muscle mass and postoperative rehabilitation.
Referral to a board-certified veterinary surgeon is recommended for either procedure. The decision between THR, FHO, or continued medical management depends on the dog's size, age, overall health, severity of disease, and available surgical expertise.
