Cataracts cloud the lens inside a dog's eye and can progress from barely noticeable to complete blindness. They develop through four stages — incipient, immature, mature and hypermature. Heredity and diabetes are leading causes in dogs. Phacoemulsification surgery is the only proven way to restore vision, and earlier assessment usually means better outcomes. Many owners notice a bluish-gray haze in their older dog's eyes and assume the worst. In most cases, that haze is nuclear sclerosis — a normal hardening and slight clouding of the lens nucleus that happens with age. Nuclear sclerosis rarely causes meaningful vision loss. Only a veterinary eye exam using a slit lamp can reliably distinguish nuclear sclerosis from a true cataract, so do not attempt to diagnose at home.
Veterinary ophthalmologists classify cataracts by how much of the lens is opaque. Each stage looks and behaves differently.
Less than about 15% of the lens is cloudy. Vision is essentially normal. Most incipient cataracts are found during routine eye exams because a dog's behavior does not change at this stage. An attentive owner might spot a tiny white fleck deep in the eye, but daily activity, play and navigation remain unaffected.
The opacity expands beyond the incipient zone but the lens is not yet fully clouded. Vision begins to decline: a dog may hesitate in dim light, misjudge distances when catching a ball, or move more cautiously at night. The immature stage is generally considered a favorable window for surgical discussion because the lens retains enough structural integrity to make the procedure technically straightforward — though the right timing depends on the individual dog and should be decided with a veterinary ophthalmologist.
The entire lens is opaque. The pupil appears milky white or gray-white to the naked eye. The dog can perceive light and dark but has little useful detail vision. Bumping into furniture, reluctance on stairs and anxiety in unfamiliar rooms are common at this stage.
Lens proteins begin to liquefy and the lens may wrinkle or shrink. This breakdown can trigger lens-induced uveitis (painful inflammation inside the eye) and secondary glaucoma (dangerously high eye pressure). Signs include squinting, tearing, redness, pawing at the face and appetite loss. The hypermature stage is a high-risk period for ophthalmic emergencies and requires urgent veterinary evaluation.
Hereditary cataracts are the most common type in dogs. Breeds with a recognized predisposition include Boston Terriers, French Bulldogs, American Cocker Spaniels, Poodles, Siberian Huskies and Golden Retrievers. Hereditary cataracts may be present at birth or appear in young adulthood — so yes, young dogs can get cataracts. The age of onset varies by breed and individual genetics.
Diabetic cataracts deserve special attention. High blood sugar causes glucose to accumulate in the lens and convert to sorbitol, which draws water into the lens fibers and causes rapid swelling and clouding. Diabetic cataracts can progress from incipient to mature within days to weeks, making regular eye checks critical for diabetic dogs.
Age-related cataracts are the most common type in senior dogs, typically appearing after age eight. They progress slowly compared with hereditary or diabetic forms, giving owners more time to plan.
Traumatic cataracts result from blunt force, penetrating injuries or other eye trauma that damages the lens capsule. Chronic uveitis, retinal disease and certain medications can also lead to secondary cataracts.

There is no single answer. Age-related cataracts may take months to years. Diabetic cataracts can reach the mature stage in days. Hereditary cataracts vary by breed. Because progression speed is unpredictable, any confirmed cataract warrants regular follow-up exams.

There is no proven eye drop, supplement or home remedy that reverses cataracts. Phacoemulsification — the same ultrasound-based technique used in human cataract surgery — is the only established treatment to restore sight.
Surgery is most commonly discussed at the immature or mature stage. The decision depends on the dog's overall health, retinal function, the rate of progression and the owner's capacity for postoperative care. A veterinary ophthalmologist is the right specialist for this conversation.
Before surgery, the ophthalmologist performs a thorough evaluation — slit-lamp exam, tonometry, ERG and ultrasound — to confirm the retina is healthy enough to benefit from lens removal. Blood work and a general health check ensure the dog can safely undergo anesthesia.
During phacoemulsification, the surgeon makes a small corneal incision (typically 2–3 mm), uses ultrasonic energy to break up the cloudy lens and aspirates the fragments. In most cases, an artificial intraocular lens is implanted to restore focus.
Reported surgical success rates are high — commonly cited around 90% or above — though outcomes depend on the stage, the presence of complications and the skill of the surgeon. Surgery carries risks including postoperative inflammation, secondary glaucoma, retinal detachment and posterior capsular opacification.

Most dogs show noticeable vision improvement within days. Full healing generally takes four to six weeks. During recovery, keep the dog away from water, mud and dusty environments.
